- Click on the appropriate PDF link below to open our Provider directory.
- On the web browser tool bar, locate the ‘search’ or 'find' option.
- Enter the key word you’re searching for and click search / find.
- To view additonal search records, scroll thru the results panel.

Attention Members:
If the provider you are intending to visit is not on our directory, please follow
the instructions below:
- Complete the
provider nomination form
- Indicate the date of your appointment in the comments section.
- Submit via e-mail to: providerservices@jmhhp.com

Every effort will be made to make sure your visit is approved. If the provider does not wish to participate with the plan, a member services representative will be in contact to advise you of the status and provide assistance in finding a provider.
